Pakistan's Silent Health Crisis

Mental health is the most neglected aspect of healthcare in Pakistan. According to the World Health Organization, approximately 24% of Pakistan's population — roughly 50 million people — experiences some form of mental health condition. Depression affects an estimated 34 million Pakistanis, anxiety disorders affect 15 million, and substance use disorders affect 6-8 million.

Yet the treatment gap is staggering. Only 1 in 4 people with a mental health condition in Pakistan receives any form of treatment. The country has just 500 trained psychiatrists for a population of 230 million — a ratio of 1 psychiatrist per 460,000 people, compared to 1 per 10,000 in developed countries.

The consequences of this neglect are severe. Pakistan's suicide rate, though underreported due to legal and social stigma, is estimated at 8-10 per 100,000 — with higher rates among young women. Mental health conditions cost the economy an estimated Rs 150 billion annually in lost productivity, healthcare costs, and social welfare burden.

Common Mental Health Conditions

Depression. Persistent feelings of sadness, hopelessness, and loss of interest in activities. Physical symptoms include changes in appetite, sleep disturbances, fatigue, and difficulty concentrating. In Pakistan, depression is often masked by physical complaints — headaches, stomachaches, and body pain — because acknowledging emotional distress carries stigma.

Anxiety disorders. Excessive worry, restlessness, and physical tension. Generalized anxiety, social anxiety, and panic attacks are common but frequently undiagnosed. Many Pakistanis experience anxiety about financial security, family honor, and social status without recognizing it as a treatable condition.

Post-Traumatic Stress Disorder (PTSD). Common among survivors of natural disasters (floods, earthquakes), conflict, and gender-based violence. The 2022 floods alone left an estimated 3 million people with trauma-related symptoms.

Substance use disorders. Pakistan faces a significant substance abuse challenge, with an estimated 6-8 million people affected. Heroin, methamphetamine (ice), and prescription drug abuse are growing concerns, particularly among young men.

Understanding the Stigma

Stigma is the single biggest barrier to mental health care in Pakistan. Several factors contribute to it:

  • Cultural norms. In Pakistani culture, mental health issues are often viewed as weakness, character flaws, or spiritual problems rather than medical conditions. The phrase "sab theek ho jayega" (everything will be fine) is often used to dismiss genuine suffering.
  • Family honor. Seeking mental health treatment can be perceived as bringing shame to the family, particularly for women. Families may discourage members from seeking help to avoid social judgment.
  • Lack of awareness. Many Pakistanis do not recognize the symptoms of common mental health conditions or understand that effective treatments exist.
  • Religious misconceptions. Some communities view mental health treatment as contrary to faith, believing that prayer alone should be sufficient. This is not supported by Islamic teachings, which encourage seeking medical treatment for all conditions.

  • Pakistan Psychological Association. Referral service for licensed psychologists nationwide
  • Telehealth services. Platforms like MindScape and Healthwire offer online therapy sessions in English and Urdu

In an emergency (suicidal thoughts, self-harm, psychotic episode), go to the nearest emergency department or call the police emergency number (15). Mental health emergencies are medical emergencies — do not wait.

What You Can Do

Start the conversation. Talking openly about mental health — with friends, family, and colleagues — is the most powerful way to reduce stigma. Share your own experiences if you are comfortable doing so.

Learn to recognize signs. If a friend or family member withdraws from social activities, shows persistent sadness, changes in sleep or appetite, or talks about feeling hopeless, reach out with compassion and encourage them to seek professional help.

Take care of your own mental health. Regular physical activity, adequate sleep, social connections, and limiting social media consumption are evidence-based strategies for maintaining mental well-being.
