Foreign suppliers, ranging from European biotechnology firms to Asian diagnostic manufacturers, refuse to process orders even when provided with official humanitarian clearance documents.
The human toll of this financial isolation falls heaviest on vulnerable patient populations requiring specialized imported therapies. Iran’s domestic pharmaceutical industry produces roughly 95 percent of the country’s basic medications, but relies on imported active pharmaceutical ingredients (APIs) and advanced biological treatments for complex conditions.
Patients suffering from Epidermolysis Bullosa (EB)—a rare genetic skin disorder known colloquially as 'butterfly disease'—have become a stark illustration of the shortage. When a Swedish medical technology firm ceased exporting specialized protective dressings to Iran under pressure from financial clearing banks, dozens of Iranian children suffered preventable complications and systemic infections.
Similar crises jeopardize care for oncology patients requiring advanced chemotherapy agents, individuals with hemophilia needing clotting factor concentrates, and organ transplant recipients who depend on specific immunosuppressive drugs to prevent tissue rejection. Local hospitals report chronic shortages of specialized contrast agents for diagnostic imaging and repair parts for foreign-manufactured radiation therapy equipment.
Tehran’s characterization of these measures as illegal economic warfare aligns with prior rulings from international bodies. In 2018, the International Court of Justice (ICJ) ordered the United States to ensure that sanctions did not impede humanitarian supplies or civil aviation safety. Despite the ICJ’s order, financial bottlenecks tightened further as additional sanctions targets were added to American regulatory blacklists.
The persistent disruption of medical imports has accelerated Iran’s push to build non-dollar financial architecture alongside regional trade partners. By utilizing bilateral currency swaps, direct barter systems, and alternative messaging mechanisms with partners across Asia and Eurasia, Tehran aims to bypass the Western banking system entirely.
Yet, developing alternative trade channels for highly regulated pharmaceuticals takes years. For patients requiring specialized therapies today, administrative workarounds offer little immediate relief against an uncompromising global financial blockade.
Although U.S. regulations technically exempt humanitarian goods, foreign banks refuse to process money transfers for Iranian importers due to fear of U.S. secondary sanctions. This financial isolation prevents pharmaceutical companies from purchasing necessary active ingredients and specialized equipment.
Esmaeil Baghaei characterized U.S. economic measures as illegal acts of state terrorism that directly endanger the lives of millions of Iranian citizens. He emphasized that Washington's financial restrictions create an artificial health crisis despite formal claims of humanitarian exemptions.
The shortages severely affect treatment for rare diseases such as Epidermolysis Bullosa, advanced chemotherapy agents for cancer patients, clotting factors for hemophilia, and specialized immunosuppressive drugs required for organ transplant recipients.
